Deadlines

Please ensure that you are aware of the published deadline for the form in which you are submitting. 

Processing can take 1-3 weeks, based on the time of the year. 

Some students are selected for verification, at random, to confirm that the data reported on your FAFSA form is accurate.  Students will be asked to provide selected documentation to our office, so that we may review it for accuracy.  If you are selected, please ensure that you submit the requested documentation in a timely manner to avoid delays in awarding.

Steps to Complete Your Forms 

  1. Please print out the form you need below and fill it out completely, including a "wet" signature (aka print it out and physically sign it) or complete electronically, when available. 

  2. You can email your form to financialaid@pikespeak.edu or turn it in at any of our Enrollment Services Centers or fax to (719) 502-2074.

  3. If you need any other forms or have any questions, please contact Financial Aid (719) 502-3000.

***Forms must be completed in blue or black ink. Failure to accurately complete these forms may result in a delay of processing or change of financial aid eligibility. Additional documentation may be requested. Read instructions carefully before completing.

Generic Form

Options to obtain tax documents required for verification.

Visit the IRS website

  1. Request online: create an account with the IRS to download your transcripts.
    • We recommend downloading the 2020/21 Record of Account Transcript, as this includes all the information we need.
    • Note: You will need a credit card, student loan, mortgage, home equity loan, or auto loan to verify your identity to create an account. If you do not have one of these, you will need to request the transcript by mail or through Form 4506-T.
  2. Request by mail: if you still live at the address listed on your 2020/21 tax return, click “Get Transcript by Mail” and complete two requests: one for the Account Transcript and one for the Return Transcript for 2020/21. The IRS will mail the transcripts to the address listed on the 2020/21 tax return.
    • Note: The IRS does not have an option to request a Record of Account Transcript through the online mail request system.
  3. If you no longer have access to mail at the address on your 2020/21 tax return, use Form 4506-T from the IRS to request a Record of Account Transcript (check box 6c) for the year that ended December 31, 2020/21 (line 9). Mail or fax Form 4506-T to the address listed on Page 2.

Use this form, if your Marital Status in 2020 is different than on the day you completed the FAFSA.  

Student Status 

If you filed a joint tax return in 2021 but you're no longer married as of the date the FAFSA was completed, you will also need to submit the following

    • Signed Copies of your 2021 Federal Tax Returns 
    • Signed copies of your new spouse's 2021 Federal Tax Returns  

If you did not file a joint tax return in 2021 but you are now married as of the date the FAFSA was completed, we will need the following: 

    • Signed Copies of your 2021 Tax Returns
    • All 2021 Student W2'S 

Parent Status 

If your parents filed a joint tax return in 2021 but are no longer married as of the date the FAFSA was completed, you will also need to submit the following:

    • Signed Copies of your parent’s 2021 Tax Returns 
    • All W2’s from the parent whose information was used to complete the FAFSA.

If your parents did not file a joint tax return in 2021 but are now married as of the date the FAFSA was completed, we will need the following: 

    • Signed copies of your parent and stepparent's 2021 Federal Tax Return. 

*Documents can be emailed to financialaid@pikespeak.edu please include your student number in all correspondence. 

Use this form if you have no earned credit with PPSC and have attended 2 or more schools between 2019-2022. 

Please note, you may be required to submit Official Transcripts from each school attended during this time period. 

UEH Form

Use this form if you have received a outside scholarship. This form will allow our office to share certain information to the scholarship donor to ensure your award is posted to your account correctly. You will need to print this form to complete it and return to Jenny.Madsen@pikespeak.edu
